Job Description

Job Summary

This position is Mon- Fri 7-3:30 or 8-4:30 pm. The Registered Nurse provides skilled nursing care in accordance with InnovAge policies and all applicable regulations to participants and performs related administrative tasks for InnovAge.

Essential Functions and Work Responsibilities

Triages participant and staff phone calls to s to determine priority of care and routes accordingly.

Schedules outside appointments for participants when necessary.

Assists in the examination, testing and treatment of participants.

Completes certification/recertification orders and discharge summaries

Implements orders written by primary care physician(s).

Instructs participants and family regarding medications and treatment instructions.

Schedules specialist and other appointments for participants and assists in hospital admissions.

Performs general nursing care on participants including administering prescribed medications and treatments.

Documents all necessary information and maintains participant medical record(s) and fulfills agency charting and reporting requirements.

Administers medications, provides training in self-administration of all non-scheduled medications and monitors medication compliance.

Provides health counseling to participants and their families on an individual or group basis.

Assists in the examination, assessment, evaluation testing and treatment of participants.

Observes, records, and reports participants condition and reaction to drugs and treatments to physicians

Triages participant and staff phone calls and concerns for appropriate follow-up and maintains a visit log of all contacts with participants.

Assesses and re-assesses participants to determine individual needs and to ensure the least restrictive environment. Assesses participants to determine change in condition that necessitates a change in level of care and coordinates the plan of care with Home Care resources.

Perform nursing care to clients in their place of residence.

Provides skilled supportive and restorative nursing care to participants as ordered, re-evaluates their care plan and health status on an on-going basis, as needed.

Administers medications, provides training in self-administration of all non-scheduled medications and monitors medication compliance.

Assist the Home Care Manager in the development of processes to support the Home Care licensed and non-licensed nursing staff.

Delegate tasks necessary to complete projects, performs audits to ensure project compliance and reports results of audits and compliance to the Home Care Manager.

Compile information related to participant satisfaction with I.H.S. staff performance and will initiate resolutions within the scope of practice and job description in collaboration with the Home Care Manager.

Ensure that all orientation check lists have been completed for newly hired Home Care staff.

Meets with newly hired staff on a regular basis to ensure that they are adjusting to the role and making progress. Reports progress to Home Care Manager for follow-up.

Ensure that quality care is delivered by home care team to site participants via periodic participant/caregiver inquiry, intermittent review of care plan, and trending of data from various reports/supervisory visits in collaboration with the Home Care Manager.

Acts as an educational resource for techs regarding skills training, education and delegation.

Attends Intake and Assessment Meetings, Family Meetings and other Planning Meetings in representation of the Home Care department.

Participates within the interdisciplinary team in the formulation of Plans of Care for InnovAge PACE program participants, as well as in other interdisciplinary team settings that plan, coordinate and monitor the care of InnovAge PACE program participants.

Job Qualifications

Associates degree in nursing

Licensure, Certification, Registration or Designation

Current State issued Registered Nurses License required.

Current CPR certification or the ability to obtain CPR certification is required.

Require personal transportation, current state issued drivers license, good driving record and auto insurance as required

Work Experience and Qualifications

A minimum of one year working with the frail or elderly is required.



  • Bachelor's degree in nursing

Licensure, Certification, Registration or Designation

  • Certification as a Gerontological Nurse

Work Experience and Qualifications

Three years of health care experience with emphasis in geriatrics

A minimum of two years of health care experience with emphasis in geriatrics and broad knowledge of health care problems that afflict the frail/elderly is highly preferred.
