Job Summary

An Occupational Therapist (OT) provides occupational therapy treatment to participants as directed by the physician orders. All participant care provided is in accordance with applicable InnovAge policies and state regulations

Essential Functions and Work Responsibilities

Functional Category: Client Care

Estimated Percent of time Spent 100%

Participant Care (Approximately 100% of Time)

Provides clinical occupational therapy evaluation and treatment on an acute, restorative and maintenance level to program participants.

Assesses condition, creates a treatment plan and discusses implementation of therapy with participant or caretaker(s).

Integrates occupational therapy treatment plan in the team Plan of Care (POC).

Assists in the ordering, inventory, distribution and maintenance of durable medical equipment for participants.

Educates participants and family members in proper care and use of various supports and exercise programs in addition to the use of durable medical equipment (DME).

Administers treatment program per referral from assessments from the interdisciplinary team (IDT)

Instructs family members, caregivers and staff in methods to maximize safety and functional abilities in performing Activities of Daily Living (ADL) with documentation in medical record notes.

Evaluates charts and reports on participants progress to appropriate staff.

Documents occupational therapy services provided and performs department record-keeping in keeping within documentation requirements.

Performs data collection, prepares reports and maintains any necessary participant statistics.

Assists in recommending policies and procedures in the area of occupational therapy.

Job Qualifications

High School Diploma or GED

Graduate of an Occupational Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA)

Licensure, Certification, Registration or Designation


National Certification by the American Occupational Therapy Association or eligible for National Registration Examination of the American Occupational Therapy Association.

Require personal transportation, current state issued drivers license, good driving record and auto insurance as required by law.

Work Experience and Qualifications


A minimum of one year experience working with the frail or elderly is required.


Two (2) years experience, preferred

Other Knowledge Skills and Abilities Required

Computer Skills

Must be computer proficient and possess experience with Microsoft Word, Excel, and Outlook.

Must be able to quickly learn specific software and new applications.
