Job Summary

Under the supervision of the Agency Manager or Administrator, the Care Supervisor ensures the proper delivery of services in accordance with contractual agreements and supervises caregiver performance as it relates to attendance, punctuality and other measurable factors.

Essential Functions and Work Responsibilities

Functional Category: Client Administration

Estimated Percent of time Spent -50%

Serves as liaison between the client, family and/or caretaker(s) and InnovAge Homecare.

Informs clients of other services offered by InnovAge Homecare that may be of use to them, as appropriate.

Determines pay and bill rates based on established guidelines and ensures that clients and/or caretakers are aware of financial conditions up front.

Performs an initial overall evaluation of clients physical and mental condition, home environment and family dynamics at the onset of service.

Responsible for the development, implementation and evaluation of clients Plans of Care.

Performs assessments of clients physical and mental condition, home environment and family dynamics on a routine basis as prescribed by established guidelines and regulations.

Ability to supervise, guide and develop client care.

Education

Required

High School Diploma or GED

Licensure, Certification, Registration or Designation

Required

Requires state issued drivers license, personal transportation, good driving record and auto insurance as required by law

Work Experience and Qualifications

Required

Two (3) years experience in the areas of gerontology and medical/social services

Two(2) years supervisor or lead experience

Current knowledge and awareness of the physical and emotional needs of seniors who are physically and/or mentally impaired and an understanding of the cares and concerns associated with the aging process.

Other Knowledge Skills and Abilities Required

Computer Skills

Must be computer proficient and possess experience with Microsoft Word, Excel, and Outlook.

Must be able to quickly learn specific software and new applications.

Mathematical/Financial Skills

Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

Able to analyze data and statistics and draw reasonable conclusions and compile accurate reports.
